Patient-focused healthcare provider opens its 16th center

Oak Street Health, a network of primary care centers that provides value-based care tailored to Medicare recipients, has announced the opening of its 16th Chicagoland center, located in the Northwest suburb of Elgin. This comes shortly after a partnership with Aurora Health Care to expand statewide coverage, opening the doors to hundreds of Medicare patients in need.

In addition to bringing jobs to the community, Oak Street Health says the new center will provide a new option for healthcare to the 12,000 seniors that live within Elgin’s boundaries. Through its commitment to preventive, personalized services, Oak Street Health has already reduced hospital admissions and readmission rates for Chicagoland’s Medicare population.

Oak Street Health operates nearly 50 primary care centers in Illinois, Indiana, Michigan, Ohio, Pennsylvania and Rhode Island and offers Medicare recipients integrated services that go far beyond primary care, including transportation to appointments, Medicare education classes, behavioral health services, community events, and more.
